Computer graphics (on the home level) were really primitive. 640 x 480 x 256 colors was considered really super high res. In terms of games, Doom was still 3 years away, and Wolfenstein 3D wasn't even out yet. Were talking a DOS-based, CGA pre-Windows 3.0 world here. In that sense, the pixelized Queensryche logo was indicative of the state-of-the-art in computer graphics for the day. Yes, they could have made it look "better", but I think the idea was to get that computerized look, to represent the beginning of the digital age, however cliched that term is today. In short, it's not a mistake, and it fits the time it came out perfectly.
